- Power of Attorney (or a certified copy)
- Living Will (or a certified copy)
- Organ donor information
- Safety Deposit Box details and key location
- DD214 – If discharged after January 1, 1950 (or a certified copy)
- DD256 - If discharged from the National Guard or Reserve
- Last Will and Testament
- Social Security number and related information
- Job, Retirement Pension, and Insurance Plan details
- Personal Life Insurance information
- American Legion Accidental Death insurance details
- Birth Certificate (or a certified copy)
- Marriage License (or a certified copy)
- Spouse’s Death Certificate, if they predeceased you
- Previous Marriage and/or Divorce records (or certified copies)
- Children’s information
- Updated details for your Obituary
- “Last Word” letters to family and friends
